If your alien registration card is lost or stolen, you must apply for a replacement card.
Application period: Within 14 days of the date of loss or theft
Documents to submit: Passport, two identical photos (as described under "New registration," above), and application form for re-issuance of the alien registration card (available at the counter).
You must also submit relevant documentation as proof of the loss or theft, such as a fire certificate or a police report of the theft.
