Procedures to be followed by Japanese and foreign residents who are residents of Nagoya City:
1. A family member must submit a death certificate (shibo todoke) issued by a doctor (and bearing the doctor's seal) to the municipal office of the deceased within seven days of the date the death becomes known. A cremation certificate (shitai
kaso kyoka-sho) will be issued after the above procedure has been completed. This includes fetuses (four months old and older, which need a death certificate called shizan todoke) lost during miscarriage.
2. Call 052-832-1750 to arrange a time for cremation (kaso) at the Yagoto Cemetery Office. You must present the cremation certificate at the office; burial is not permitted in Nagoya City. For Japanese and foreign residents who are residents of
Nagoya City cremation at Yagoto Cemetery is free of charge.
3. Within 14 days of learning of the death, return the alien registration card and national health insurance certificate, if any, of the deceased. Both National health insurance and Employee's Health Insurance will cover the cremation or funeral fees.
4. Report the death to the embassy or consulate of the deceased person.

Office. Phone 0568-28-1049 for shipment by air or 052-398-4225 for shipment by sea. Please contact the embassy or consulate of the deceased to learn the necessary procedures for the repatriation of the remains.
