Vaccinations will be provided in Nagoya City to prevent the spread of COVID-19. Two doses will be administered.
Those registered as residents of Nagoya City at the time their vaccination is to be administered
*Exceptions may be made for persons currently in long-term hospitalization, confinement to a facility, or other unavoidable circumstances, to receive the vaccination in another municipality.
Nagoya International Center will help you to find a vaccination venue and make a reservation.
- NIC can help to find a medical facility that provides vaccinations, or a group vaccination venue.
- NIC can help with reservations for group vaccination venues. As the explanation is difficult by phone, please come to NIC in person with your vaccination coupon.
- NIC can provide information on clinics where you can get the vaccination. If you need interpreting assistance when making a reservation, please use the NIC Trio-phone service. *Vaccinations will be administered after information has been provided thoroughly, and only with the consent of the individual to be vaccinated.

*Children turning 12 after 1 August will be sent a vaccination coupon at the beginning of the month following their birth month (i.e. after they have turned 12).

Procedure for receiving the vaccination
(1) A vaccination coupon will be sent to each eligible individual by post.
You will be sent a vaccination coupon in an envelope like this:

(2) Once you receive a vaccination coupon, make a reservation to receive the vaccination.
①For a vaccination at a group vaccination venue:
Make a reservation via the Call Center or online. You can also inquire about group vaccination venues.
*Group vaccination venue: venue where multiple people can be vaccinated at once, administered by local government
*Some employers may provide workplace vaccinations for employees. Workplace vaccinations are not provided at group vaccination venues referred to here. For details of venues for workplace vaccinations, please refer to information on workplace vaccinations as provided by your employer.

Vaccination Certificate of COVID-19 (for Nagoya City residents)
The City of Nagoya will issue a Vaccination Certificate of COVID-19 (shingata koronauirusu wakuchin sesshu shomeisho / 新型コロナウイルスワクチン接種証明書) to residents who are required to provide proof that they have been administered a COVID-19 vaccine in order to travel overseas. This certificate is not required within Japan, where the Certificate of Vaccination for COVID-19 (attached to one's Vaccination Coupon) is sufficient proof of vaccination.

How to apply:
Applications should be submitted by post (postage charges apply).
(1)Send the required documents to Nagoya City Hall.

(2)The City of Nagoya will confirm the content of the submitted documents.
(3)The City of Nagoya will issue your Vaccination Certificate of COVID-19.
*It will take around 3 or 4 days after receipt of the application for the Certificate to be issued and sent. A large volume of applications, errors in submitted documents, or insufficient documents will cause a delay in the issuance of the Certificate.
*If you are departing Japan immediately, and do not have sufficient time for the standard application process, please contact the Call Center (050-3135-2252).
・Copy of applicant's passport
・Copies of the following sections of your Vaccination Coupon:
1. Coupon number
2. Certificate of Vaccination for COVID-19 (attached to one's Vaccination Coupon)
*Copies of both sections should be provided on one sheet of paper
・A stamped, self-addressed envelope
・A copy of a document to verify the applicant's address ('My Number' Individual Number Card, Residence Card, driver's license, etc.)
*If you are applying on behalf of another person, a Letter of Proxy (ininjo / 委任状), signed by the person, is required.
