The vaccine for the Omicron variant helps prevent infection, and onset of COVID-19 while also lessening the severity of symptoms.
Overview
Who is eligible?
Nagoya residents aged 12 and older who have had their first round, 1st and 2nd dose, of vaccines.
You may receive the booster shot starting after three months from your most recent vaccination.
*Currently, the vaccine for the Omicron variant is limited to one per person.

Vaccination Voucher

- A vaccination voucher for the 3rd, 4th, or 5th vaccination issued by the City of Nagoya is required.
- If you have not received your 3rd or 4th dose and already have an unused voucher, you may use that voucher to receive the vaccine for the Omicron variant.
You will not receive the same vaccination voucher twice.
If you have lost or damaged your vaccination voucher, you can apply to the City to re-issue a new voucher (see Application for Issuance or Re-issuance of Vaccination Voucher below). - If you have already received your 2nd, 3rd, or 4th vaccination and have not yet received your next vaccination voucher, it will be sent three months after your most recent vaccination.

Confirm the shipping schedule for vaccination vouchers via the official website of the City of Nagoya (Japanese). If you do not receive a voucher within one week of the scheduled shipping date, please apply for a vaccination voucher (see Application for Issuance or Re-issuance of Vaccination Voucher below).
Steps for Vaccination
Vaccinations are carried out at medical institutions and mass vaccination venues. In principle, you must make an appointment to receive a vaccination.

Application for Issuance or Re-issuance of Vaccination Voucher
A vaccination voucher issued by the City of Nagoya is required.
If you do not have, or you have lost or damaged your vaccination voucher, you will need to submit an application to the City to issue, or reissue, a voucher.
Those who fall under the following categories are eligible to apply for reissuance of a vaccination voucher.

- Those who have received their recent vaccination outside of Japan.*
- Those who received their recent vaccination through the vaccination program for Japanese citizens living outside of Japan.*
- Those who received their recent vaccination from the U.S. Forces stationed in Japan. *
- Those who received their recent vaccination as part of a clinical trial for a pharmaceutical manufacturer.*
- Those who wish to have the voucher sent to an address not listed in the resident record (住民票).
- Those who have not received a voucher within one week of the scheduled shipping date.
* Only if you have received the Pfizer, Moderna, AstraZeneca, Takeda (Novavax) or Janssen Pharma vaccines.
* Fosun Pharma, COMIRNATY from BioNTech, and Covishield and COVOVAX from the Serum Institute of India are also included.
* For Janssen Pharma vaccines: one vaccination outside of Japan is considered as two vaccinations in Japan, and two vaccinations outside of Japan equal to three vaccinations in Japan.
Those who fall under the following categories must apply for the reissuance of a vaccination voucher.

- Those whose vaccination voucher is lost or damaged
- Those who used a voucher to consult with a medical physician (preliminary exam), not to receive vaccination.

Documents to be submitted
Application for (Re) Issuance of COVID-19 Vaccination Voucher (3rd, 4th, 5th) and Changing Mailing Address
A copy of at least one item from the list to confirm recent vaccination:
・Certification of completion of vaccination
Vaccination record
Vaccination certificate (vaccine passport)
・Vaccination voucher from another municipality with record of recent dose
Note: If you do not have any of the above written documents, you will need a copy of your ID i.e.: driver's license, health insurance card, My Number Card, etc.
Form
1st round vaccinations for those aged 12 and older, 1st and 2nd dose, will continue to be available.
The first and second doses will not be vaccines for the Omicron variant.
If you do not have a vaccination voucher, you will need to submit an application to the City to issue, or re-issue, a voucher.
